Pre-Application for Comprehensive Heart Attack Center Certification

The Comprehensive Heart Attack Center certification officially goes live on July 1st, 2022, meaning the official application and onsite reviews will be available at that time.

This pre-application is an abbreviated questionnaire that can be submitted before July 1st, that allows an organization to identify a ready date for their initial onsite review and enter the scheduling queue. This is ideal for organizations that hope to be reviewed during the first few months after launch. The earlier you submit the pre-application, the earlier we begin the process of scheduling the review according to your ready date.
